Not spiked ammo.
My take is as follows:
The gunner forgot that the launcher doesn't lie in a direct line along the sight picture... and fired it at point-blank range into whatever that wall he's meant to be shooting over, is made from, essentially capping the RPG inside the tube. Gas pressure from the main booster charge then caused the tube to rupture, cutting him to pieces.
I don't think that the warhead itself exploded because the guy kneeling close with the periscope would've been fragge More..

Doesn't matter how good you clean it when there are circulated dummy rounds that explode on purpose. Not sure if that's what happened as it's hard to tell, he may have been hit as he fired, but there are such things as dummy rounds that are aimed to kill or maim the shooter of RPG's, mortars and even AK's. Been happening since WWII.

The guy whose death you're cheering might be some religiously moderate guy who wants nothing more than democracy, an end to a violent and extremely repressive dictatorship, and a future for his family that doesn't involve massive unemployment and a dependency on government food subsidies. I suggest you to wait for the iRpg that Apple will launch soon. It will come with HD camera so you can record your own dead and upload it directly in LL. You can call your friends while shooting and really useful feature is the snackbar-tunes where you can download and play a wide collection of snackbar chanting.
