Agreed. The person who should get fired is either the person that designed the shelving or the person(s) who put it together.
Most warehouses will also have stops at the base of the shelving to prevent that from happening.

The racks would have probably handled it had they not been assembled incorrectly.
It looks like there were two weak points.
First, they had built entryways through the racks, which are not a good idea. Without these the legs and racks would have been supported better, and the hit would have been unlikely to cause a collapse.
Second, it also seems that nobody bothered to attach the legs to the floor, which is another big no-no. Even with the entryways the racks might have survived if the legs More..

Looks to me like he asked first forklift driver to get it down for him.He was probably told 'get it yer fuckin self'or words to that effect.He did,and now we see the results.First fork driver had it spot on.Do nothing but backwards and forwards.
