This doctor is the biggest moron that I've ever seen in my life. If someone appears to be seriously injured, provide first aid, cover him up if his skin feels cold, or remove clothing and fan him if he's burning up. However, do not elevate the legs and d'ont touch the part with hands only apply it with Sterile Compress and d'ont leave it open no longer than two hours, as this will increase bleeding and make it more difficult for the victim to breathe.

I think the doctor was poking the wound to see if there was more shrapnel, not to provoke his agony. His face was badly torn up though...

You're correct. The patient isn't bleeding from the wound, so it needs to be cleaned and cleared before long-term care can be started. There's likely a major shortage of standard surgical drugs available (i.e. anesthetics), so the medic has to make do. Funny how all the rest of the LL "doctors" don't seem to be recalling the basics of field medicine.
