He shouldn't have made that jump. You could clearly see that she was panicking and trying to hold back the jump. And it looks like she almost wriggled out of those straps right before ( what looked like a forced) jump.

There were a number of problems on that jump. First the side spin and the harness problem. One of the instructor's jobs is to make sure the harness is adjusted properly. Good job by the video guy to try to help remedy the situation.
