looks like he threw it AND primed the grenade at the same time, by throwing the grenade with his finger on the string primer. this looks like it caused a 'tug' during the throw, which pulled it off on a tangent and ....the results speak for themselves.

I had a female cousin that is a Marine. During boot camp they were training w/ live grenades. She is one of those people that writes with her left and throws with her right...Well there was some confusion and she ended up throwing the grenade straight up in the air...It landed in their little bunker thing. Her instructor took action and saved them both!

The joke about "drop the grenade, throw the pin", does actually happen. People don't always know how they'll respond in those situations, maybe they're terrified of the explosive in their hand and trying not to show it.
Instructors have died throwing the kid out of the way to save them after they dropped the grenade and tossed the pin.
