Tourist Bus Too Tall For Tunnel
A Taiwan tourist bus carrying passengers from the mainland city of Guangzhou crashed into the roof of a tunnel it was clearly too large for, injuring 15 of the 24 tourist passengers, two of which were in intensive care.
The accident occurred around 1pm on the 24th of August. It is believed the driver failed to consider the height of the tunnel.

The driver forgot. It happens. I would put a restrictive pipe (plastic) for 100 meters from the entrance. And the offender would pay for proto plastic pipe, not the bus and of course the emergency services.
