From the confusion in the beginning of where the target would be and when, and including the dis jointed attack formation and lack of strategic planning, to the failure to actually engage a target succesfully. How about just when the men argue about the detonator not working and the guy pushes the button and it blows up? setting up an engagment point where there are women and children? A total lack of discipline
