M4 Carbine Jams on soldier Over and Over in the Middle of a Fire-Fight

You can hear the bullets whipping past him at least twice. He can't make this rifle work.
My guess is worn or dirty extractor - dirty chamber, or all of the above. Anybody have their own guess?
