It was a private entrance to a company with a separate gate but it was closed because the company was not in business. She was dead after trapped there for 7 days later before being found.

Cases like that is not uncommon. We used to have the situation like that before when our company rented the whole floor as office space and installed a gate at the elevator door for security reasons. The 1-2 ft wide gap between our company's gate and the elevator door became a trap when someone stepped out of the elevator right after the slide door opened. I told them to change the gate to a sliding fence so people trapped in the gap could strech their arms through the holes to press the elevator button.
