The reason why he doesn't feel pain is because it is a cyst and not an abscess. A cyst is a sac in a non preformed cavity filled with a sterile substance whereas an abscess is a sac in a non preformed cavity filled with pus. A cyst can become infected and develop into an abscess but as long as it is not it wont really hurt.
This is freaking disgusting by the way, I just can't stand it when they pop like this. I saw a doctor once whose face got drenched in brown liquid from a bartholins cyst on More..

Goodness, aren't some people "know it alls". Speaking as one who has had several similar sebaceous cysts removed and a son who is surgeon I am reasonably well qualified to comment on the vid. Gross for sure, no arguments there. But if you don't remove the sac (as someone else posted) then it will just get re-infected and effectively grow back. In fact removing the entire sac is tricky and I can only advise people choose their surgeon carefully. Doing this at home as in the video i More..

This lady knew what she was doing because not only did she fully drain the cyst but she also dug out the core. A lot of people who don't know what they're doing always neglect that. You can see at the end she scraped the inside with those scissors which would scrape off the internal sack of the cyst and that bloody stuff that came out at the end is the pieces from that.

These home extractions, in the vast majority of cases, including this one, never use a large enough incision. That cyst will probably grow back because of incomplete sac removal.
