I agree with your sentiment, but it's much more likely that this is the result of an abscess secondary to some kind of skin infection than maliciousness. These kinds of lesions come into the hospital all the time.

It's tissue loss, necrosis and infection of the scalp. You wouldn't have to dig very deep to hit skull though.
What's exposed is muscle and granulation tissue.

I don't know. Dogs have pretty amazing immune systems. I'm surprised at the things that don't kill them. Not to say it won't, just that it's hard to make that call.
