A driver of a forklift had his view obstructed by a large load, and didn't see another man
walking in front of him. The man was crushed to death by the full weight of the forklift and it's load.

This hit home with me, as I used to be a forklift mechanic for almost 10 years, and know how heavy and deadly these things are.
