Report says pilot not wearing helmet in crash into Lesser Slave Lake

A report into the fatal crash of a firefighting helicopter says the
pilot was not wearing a flight helmet when the aircraft went into the
waters of Lesser Slave Lake.

The Transportation Safety Board report says the pilot suffered a severe head injury in the May 20 crash.

Bell 212 helicopter was picking up loads of water from the lake with a
big bucket attached by a line to the aircraft and dumping it on

The report says the winds were so calm the water looked like glass, reducing the pilot's depth perception.

say he flew too low and the bucket went into the lake before the
helicopter was in a hover, causing it to crash violently into the water.

The report also says a mechanism that would have allowed the pilot to jettison the bucket had been electrically disarmed.
